Welcome to Central Ohio Aesthetics Clinic. By accessing our services, website, or visiting our clinic, you agree to be bound by the following Terms and Services. Please read them carefully.

  1. General Information

Central Ohio Aesthetics Clinic, LLC (“COAC,” “we,” “our,” or “us”) provides aesthetic, wellness, and medical services including but not limited to injectables, laser treatments, hormone replacement therapy, microneedling, hair restoration, PRP (platelet-rich plasma), weight management programs, facials, skincare, and supplements. All services are performed by licensed and qualified medical professionals.

  1. Medical Disclaimer

Our services are elective, and results may vary from person to person. Treatment recommendations and outcomes are based on individual consultations. Nothing on our website or social media is intended as medical advice or a substitute for a consultation with a licensed provider.

  1. Accuracy of Information

Patients must provide accurate, complete, and truthful information about their health, medical history, allergies, medications, and prior treatments. Falsified or omitted information can lead to adverse outcomes and may result in termination of care.

  1. Eligibility

Patients must be 18 years or older or have written consent from a parent or legal guardian to receive treatment. We reserve the right to decline treatment based on medical history, contraindications, or if the requested service is deemed clinically inappropriate.

  1. Appointments & Cancellation Policy

We value your time and ours. Please arrive 10 minutes early for your appointment. You are required to provide at least 24 hours’ notice if you need to cancel or reschedule an appointment. Failure to do so will result in a cancellation fee of $50 for each missed appointment. “No-show” appointments will incur a fee equal to the full cost of the scheduled treatment. Repeated cancellations or no-shows may result in being discharged from Central Ohio Aesthetics Clinic’s care.

  1. Payment for Services

All services rendered by Central Ohio Aesthetics Clinic are to be paid in full at the time of service unless other arrangements are made in advance. Accepted forms of payment include credit cards, debit cards, cash, and CareCredit.

  1. Insurance

The Clinic does not accept insurance for aesthetic treatments. Aesthetic services provided are generally considered elective and are not covered by health insurance plans. You are responsible for all fees associated with services rendered. Central Ohio Aesthetics Clinic does not bill insurance companies, and you agree to pay in full at the time of service unless specified otherwise by the provider.

  1. Payment Plans and Financing

If you choose to use a financing option (e.g., CareCredit), you agree to abide by the terms and conditions of the financing provider. The Clinic is not responsible for any issues or disputes related to third-party financing. You must confirm your financing approval prior to the procedure date.

  1. Non-Refundable Fees

All sales are final. No refunds will be issued for services already rendered or for partially completed treatment packages. This includes, but is not limited to, consultations, Botox, dermal fillers, laser treatments, wellness services, and other aesthetic procedures.

Pre-paid amounts are also non-refundable, except in rare cases where the Clinic is unable to provide a service due to extraordinary circumstances. Any such exceptions are made solely at the discretion of the Clinic.

  1. Late Payments

In the event of a payment default, Central Ohio Aesthetics Clinic reserves the right to charge a late fee of 15% for overdue balances each day. If an account remains unpaid for more than 5 days, it may be sent to collections, and you will be responsible for all associated collection costs, including legal fees if applicable.

  1. Estimated Treatment Costs

While the Clinic makes every effort to provide accurate treatment cost estimates, the actual cost of services may vary depending on the specific procedures performed. A final invoice will be provided at the time of service. If you have any questions about costs, it is the patient’s responsibility to ask for clarification before receiving treatment.

  1. Price Changes

Central Ohio Aesthetics Clinic reserves the right to adjust pricing for services at any time. Price changes will be communicated to you in advance, and the rates at the time of treatment will apply to the services rendered.

  1. Gift Cards and Promotions

Promotional offers and gift cards must be presented prior to service and may not be combined with other discounts unless stated. Expiration dates and terms apply to all promotional items.

  1. Privacy and Confidentiality

Your privacy is important to us. All patient records are kept confidential in accordance with HIPAA regulations. By signing our patient intake forms, you agree to the storage and use of your information for treatment, payment, and healthcare operations.

  1. Consent for Treatment

You acknowledge that you have been provided with an explanation of the treatments to be administered, the risks and benefits associated with them, and the costs involved. You agree to the recommended services and authorize Central Ohio Aesthetics Clinic to proceed with treatment upon payment in full.

  1. Adherence to Instructions

Patients must follow all pre- and post-treatment instructions and participate in follow-up appointments as recommended by COAC providers. Failure to adhere to medical guidance may compromise results and patient safety.

  1. Results and Expectations

We strive to deliver high-quality care and results. However, aesthetic treatments are not an exact science, and results vary. No guarantees are made regarding outcomes. Follow-up appointments may be necessary to achieve optimal results.

  1. Right to Refuse Service

We reserve the right to refuse service to anyone who displays inappropriate behavior, violates our policies, or is not a candidate for treatment due to health or safety concerns.

  1. Compliance with Laws

COAC and its staff follow all applicable federal, state, and local laws and regulations governing the practice of medicine, aesthetics, and wellness services.

  1. Key Clauses

Account Registration (If Applicable):
Some services may require patients to register an online account with accurate personal and health information. Patients are responsible for maintaining the confidentiality of login credentials.

Disclaimer of Liability:
While we strive to ensure safe and effective outcomes, COAC is not liable for complications, adverse reactions, or dissatisfaction with results. Elective aesthetic treatments carry inherent risks that are disclosed in informed consent documents.

Limitation of Warranty:
We do not guarantee specific results from any service. Individual outcomes may vary based on numerous factors including patient compliance, genetics, and response to treatment.

Termination of Services:
Either the patient or COAC may terminate the relationship at any time. COAC reserves the right to decline or discontinue care for non-compliance, inappropriate behavior, unpaid bills, or safety concerns.

Governing Law:
These Terms and Services shall be governed by and interpreted under the laws of the State of Ohio. Any disputes will be subject to the exclusive jurisdiction of courts located in Delaware County, Ohio.

Intellectual Property:
All content on our website, social media, brochures, and educational materials—including logos, service names, images, and text—is the intellectual property of Central Ohio Aesthetics Clinic and may not be copied or reproduced without express written permission.

Indemnification:
By using our services, you agree to indemnify and hold harmless Central Ohio Aesthetics Clinic, its staff, affiliates, and contractors from any claims, damages, or legal actions arising from your use of our services, failure to follow medical advice, or breach of these Terms.

  1. Modifications to Terms

Central Ohio Aesthetics Clinic reserves the right to modify these Terms and Services at any time. Updated terms will be posted on our website, and your continued use of services constitutes acceptance of those changes.
